We are pleased to offer an in-office dental benefit program for our patients who do not currently have dental coverage. This plan allows our patients to receive optimal dental care while maintaining their oral health.
Dental Services Include:
- Examinations (two per 12-month period)
- X-rays (as needed – does not include CBCT scans)
- Cleanings (two per 12-month period: Two to Four Perio Cleanings)
- Diagnosis
- Cosmetic consultation
- 20% discount on all other services
Pricing:
- $375.00 – Ages 14+
- $275.00 – Ages up to 13
- $525.00 – PERIO PLP
Advantages of the Patient Loyalty Program (PLP) include NO deductibles, exclusions, or waiting periods. The cost of the PLP is less than the cost of your annually prescribed preventive care and provides a 20% fee reduction on needed services to enrollees. Benefits are provided for one year from the date the plan is purchased. The Program is for you and is not transferable and nonrefundable. PLP Plan cannot be combined with any other coupons, cash discounts, or promotions.
